Commonpoint Queens is Hiring a Psychiatric Nurse Practitioner Near Queens, NY
- Health screenings and monitoring
- Providing psychiatric evaluations and ongoing medication.
- Participating in a weekly case conference meeting with the treatment team.
- Maintain contact with and participate in supervision with the Psychiatrist.
- Maintain updated clinical records in Electronic Health Records
- Conduct nursing assessments for home-based patients for initial intake and continued care
- Process medication requests and prior authorization requests
- Provide patient education and blood pressure screening for groups and individuals
- Work with program psychiatrist, RN, and social workers to establish and implement a plan of care
- Obtain outside medical records from outside providers
- Conduct telephone triage (during office hours)
